A “Measles Message" from Dr. Jacob Mirman

“With the recent outbreaks of measles in Texas and other parts of the country, many people are understandably concerned and want to protect themselves,” says Dr. Jacob Mirman. “I listened to some doctors on the radio talking about the issue, and they recommended booster shots. And while the shot is indeed a great way to protect yourself, this is not the only option available to make sure you are protected.”

“Patients can come into Life Medical for a simple blood test to determine whether they are already immune. Most people in this country are immune already because they received the shot in the past. I see no reason to get another shot if you are immune and would recommend against it if it is not needed. This measles blood test may be covered by insurance, but even if it is not, the cost to pay on a cash basis at our clinic is very reasonable; around $50.”

“We can check immunity to other conditions as well, like Hepatitis B, chicken pox, mumps, and German measles. If there is any concern, we can discuss and order appropriate tests.”

Greetings from Life Medical and Minnesota Medical & Rehabilitative Services LLC!

Our ongoing dedication to your health and well-being includes maintaining clear communication about any changes that may impact your interaction with us.

We are excited to announce that we have updated our Revenue Cycle Management (RCM) system to a new platform called Athelas Insights.

This enhancement is part of our ongoing efforts to streamline our administrative and billing processes and improve your experience with us.

What does this mean for you?

As of March 2024, we started using the Athelas Insights system for billing. You may notice some changes as a result:

  • Patient statements: The design of your patient statement will change. Our new system generates a different layout, you will always see our logo and name. Although the phrase ‘Powered by Athelas’ will also appear in the statement, this statement is still from us.

PLEASE NOTE: On approximately February 22, 2024, a serious event regarding Change Healthcare (a major claims processor) has resulted in significant processing delays of insurance claims. Due to this unforeseen circumstance, there were delays in providing accurate statements reflecting your financial responsibility. For any questions, please do not hesitate to reach out to our Billing Department at (952)-933-8900 or email us at billing.questions@lifemedical.us.

  • Online Payments: If you pay your bills online, you will now be directed to a new secure payment portal. We will provide the link to this portal in your statement and via a text message sent from +1 (833) 946-2331. An example of a text message from us would look like: “Hi [Your Name], you have a payment request from Life Medical. If you do not want to receive further text messages from us, reply “OPTOUT”

  • Email Communication: Your billing statements will be emailed from noreply@athelas.com. Please add this email to your trusted contacts to ensure receipt of all statements.

  • Payment History: Please note that any outstanding balance for dates of service prior to March 2024 is still due and will be presented in a separate statement from our previous system.

Please rest assured that these changes are aimed at enhancing our services and making the billing process more transparent and straightforward for you. Our team has been trained and is ready to help you navigate these changes.

We appreciate your understanding and cooperation during this transition. If you have any questions, please do not hesitate to contact our office at (952) 933-8900 or reach out to us at info@lifemedical.us

Learn how USCIS Form N-648 helps individuals with medical disabilities qualify for U.S. citizenship. Life Medical provides expert N-648 evaluations in St. Louis Park, MN.
December 19, 2025
IMPORTANT CLARIFICATION ABOUT FORM N-648 Form N-648 (Medical Certification for Disability Exceptions) allows certain applicants to request an exemption from the English and/or civics requirements for U.S. naturalization due to a qualifying medical condition. A key point to understand: Any licensed medical doctor (MD or DO) or clinical psychologist may complete Form N-648. The provider does NOT need to be a USCIS-designated civil surgeon to complete Form N-648. Civil surgeon certification is only required for Form I-693 (Report of Medical Examination and Vaccination Record) and does not apply to N-648 disability waivers. What matters most is that the evaluating provider: Is properly licensed Has sufficient medical expertise Clearly explains how the patient’s condition prevents them from meeting naturalization exam requirements Completes the form accurately and in accordance with USCIS standards WHAT IS FORM N-648 AND WHY DOES IT MATTER? Becoming a U.S. citizen requires passing a naturalization exam that includes English proficiency and U.S. civics knowledge. For some individuals, physical, developmental, or mental impairments make it impossible to meet these requirements. Form N-648 provides a lawful medical exemption for applicants whose condition prevents them from learning or demonstrating the required knowledge. Approval depends on clear, thorough medical documentation , not on the provider being a civil surgeon. WHO MAY QUALIFY FOR AN N-648 MEDICAL WAIVER? USCIS requires evidence of a legitimate medical condition that directly interferes with the ability to meet exam requirements. Common qualifying conditions include: Dementia or age-related cognitive decline Traumatic brain injury Stroke or cerebrovascular disease Intellectual or developmental disabilities PTSD, schizophrenia, or severe depression Neurological conditions affecting memory or language Medication side effects that impair cognitive function Each case is evaluated individually. The critical factor is demonstrating the functional impact of the condition on learning, memory, or communication. WHY THE QUALITY OF THE MEDICAL EVALUATION MATTERS USCIS officers review N-648 forms carefully and often critically. Applications may be denied if: The explanation is vague or overly technical The connection between the diagnosis and exam limitations is unclear Documentation does not adequately support the condition The form fails to anticipate common USCIS concerns A well-prepared evaluation explains complex medical issues in plain language and directly addresses how the condition prevents exam completion.
